Qualifications

• Licensure: New York State license with current RN registration.
• Certified by NYS Education Department as Nurse Practitioner with current registration.
• Has completed NP education including a master’s program or post-master NP certificate.
• Collaborative Practice Agreement with a physician if fewer than 3600 hours of NP practice.
• National Board Certification in Advanced Practice Nursing required for NPs delivering professional services.

Responsibilities

• Assesses health status of patients/families by history and physical examination.
• Initiates and orders selected procedures and therapies; prescribes medications as appropriate.
• Educates and counsels on health promotion, disease prevention, and management of acute/chronic illnesses.
• Documents findings and plan of care in the medical record.
• Collaborates with physician per established standards, policies, and protocols.
• Maintains patient confidentiality and adheres to HIPAA guidelines.
• Develops and updates the plan of care based on diagnoses and patient needs.
• Participates in FPPE/OPPE as part of credentialing.
