Job summary

Hackensack Meridian Health is seeking an Oncology Nurse Navigator (ONN) to guide patients from pre-diagnosis through the treatment continuum, coordinating care with the multidisciplinary team.

The ONN collaborates with PCPs, surgeons, oncologists, nutrition and social work to facilitate timely access to services and referrals for financial counseling and supportive care throughout the cancer journey.

Qualifications

• Bachelor of Science in Nursing (BSN) (Note: this applies to team members hired on or after March 2024)
• Minimum of two years of experience in nursing
• Excellent written and verbal communication skills
• Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms

Responsibilities

• Navigate the patient\'s journey from pre-diagnosis through the treatment continuum in collaboration with the multidisciplinary team
• Support oncology patients and facilitate access and timeliness to care
• Make referrals as applicable (financial counseling, nutrition, social work)

Under the direction of the local leadership, the Oncology Nurse Navigator (ONN) navigates the patient's journey (starting with pre diagnosis) and continues throughout the treatment continuum. In collaboration with the multidisciplinary team, which may include PCP's, surgeons, GYN, GI, oncologists, etc and the entire healthcare team, the ONN supports the oncology patient and facilitates access and timeliness to care. The ONN makes referrals as applicable (not limited to) financial counseling, nutrition, social worker.

Education, Knowledge, Skills and Abilities Required:

• Bachelor of Science in Nursing (BSN) (Note: this applies to team members hired on or after March 2024)
