In collaboration with physicians, nurses, social workers, and nutritionists, the navigator facilitates access to timely treatment, referrals for financial counseling, and supportive services to improve patient outcomes and experience.

Responsibilities

• Navigate the patient's journey from pre-diagnosis through treatment continuum in collaboration with the multidisciplinary team.
• Coordinate referrals to financial counseling, nutrition, social work and other supportive services as applicable.
• Facilitate access to timely care and communicate with patients and families to guide them through the oncology care pathway.
