Job summary

Beth Israel Lahey Health's Cancer Center Nurse Navigator serves as the primary contact for patients and referring physicians, guiding ambulatory oncology care at BIDMC sites and during transitions to BILH Cancer Center locations. Qualifications

• Bachelor's degree required.
• Registered Nurse license required.
• American Heart Association – Basic Life Support Certificate required.
• 3–5 years related clinical medical-surgical experience.
• Strong analytic skills with quality and data analysis.
• Excellent interpersonal skills and ability to work with diverse disciplines.
• Experience with computer systems including MS Office applications (Outlook, Word, Excel, PowerPoint or Access).
• OCN/Patient Navigator certification preferred.

Responsibilities

• Initiates communication with patients after a positive finding and guides them through the health care system.
• Provides health education to individuals and groups and develops language-specific materials.
• Identifies care team members and builds relationships across departments.
• Assists with questions about payment for services and arranges patient transportation when needed.
• Tracks interventions and outcomes, participates in medication reconciliation during transitions of care.
