Beth Israel Lahey Health in Burlington, MA is seeking a Nurse Navigator in Hematology/Oncology. This full-time Day shift role offers four 10-hour shifts (7:00am–5:30pm) and serves as the first contact for referrals, coordinating patient care from initial contact through diagnosis, treatment and follow-up.

You will work with the provider team to navigate same-day clinic visits, support triage, and help prevent unnecessary ER visits or hospital admissions, aligning with professional practice

Qualifications

• Graduate of an approved nursing program with a BSN.
• Current RN license to practice in Massachusetts and BLS certification.
• Hematology/Oncology or acute care experience preferred.

Responsibilities

• Serve as the first point of contact for incoming referrals and coordinate patient care.
• Navigate patients from initial contact through diagnosis, treatment and follow-up.
• Cover the telephone triage line to assist with symptom management and same-day visits.
• Collaborate with physicians and ambulatory staff to plan and coordinate care.
