Beth Israel Lahey Health is seeking a Nurse Navigator in Hematology/Oncology to guide patients from referral through diagnosis, treatment and follow-up. This role coordinates same-day clinics, triage calls, and symptom management, aiming to minimize ER visits and hospital admissions.

Under supervision of the Nurse Navigator Lead, you will work with physicians and ambulatory staff to ensure safe, coordinated care aligned with nursing standards and MA regulations.

Qualificações

• Graduate of an approved nursing program with a Bachelor's degree.
• Current Massachusetts nursing license and BLS certification.
• Strong clinical and interpersonal skills for patient care coordination.

Responsabilidades

• Act as first contact for referrals and coordinate patient care across the continuum.
• Navigate treatment plans with the provider team to ensure access and timely appointments.
• Support symptom management, triage calls, and same-day clinic scheduling.
