Beth Israel Lahey Health is seeking a Breast Navigator for the Hoffman Breast Center to guide patients through the continuum of breast health care, from screening to treatment and follow-up. You will coordinate imaging, biopsies, consultations, and surgeries while serving as the primary liaison among patients, families, and the care team.

Qualifications

• Bachelor's degree preferred.
• Breast Health Navigator Certification preferred.
• Minimum 3-5 years of clinical nursing experience in women's health or breast care preferred.
• Strong communication and counseling skills.
• Proficiency in EMR systems and Microsoft Office.

Responsabilités

• Serve as the primary contact for patients with abnormal imaging results or cancer diagnosis.
• Coordinate scheduling for diagnostic imaging, biopsies, consultations, and surgical procedures.
• Review patient medical history and communicate relevance to surgical evaluation.
• Act as liaison between patient, surgeon and primary team.
• Facilitate communication with health care staff to ensure proper workup.
• Track patient progress and outcomes to ensure continuity of care.
• Assist with obtaining outside images.
• Educate patients and families about imaging findings and treatment options.
• Refer high-risk patients to appropriate clinics.
• Participate in interdisciplinary team meetings and case reviews.
• Build relationships with internal departments and external providers.
• Contribute to quality improvement initiatives and data analysis.
• Contribute to development of patient care pathways.
