• Graduate of an approved school of nursing; BSN preferred.
• Minimum of five years of healthcare experience; acute care preferred.
• Nurse navigation experience is preferred.

Responsabilidades

• Review patient records within 12–24 hours after admission to assess needs.
• Interview patients/families to evaluate social, emotional, and financial factors.
• Coordinate post-hospital plans with care teams for continuing care.
• Identify and address resource barriers to ensure timely care.
• Maintain documentation and report quality indicators.
• Provide education to staff about case management roles.
• Maintain confidentiality and uphold Unity processes.
