Job summary

You will assess, plan, implement, and evaluate individualized care using the nursing process, acting as the primary clinical contact for patients across sites.

Qualifications

• RN license in NY required.
• BLS certification or eligibility required.
• Bachelor’s degree in Nursing with 1 year oncology/medical-surgical or critical care experience, or 3 years in these areas with a BSN.
• Associate’s degree in Nursing with 3–5 years in oncology/medical-surgical/acute care.

Responsibilities

• Coordinate multidisciplinary and multisite care.
• Educate patients, families, and caregivers about care plans.
• Communicate effectively with providers and care team across sites.
• Serve as central point of contact for patient and caregiver inquiries.
• Navigate patient care to ensure timely access and adherence to plans.
