Silver Cross Hospital is seeking a Registered Nurse to coordinate patient care through the disease process, acting as a clinical liaison across medical services and clinical departments. Assess needs and manage symptoms before and after treatment.

The role emphasizes discharge planning with families, education on disease and treatments, and collaboration with oncologists, social workers, and home care teams to ensure understanding and adherence to care plans.

Qualifications

• Registered Nurse with a current Illinois license and BSN degree.
• Oncology Nursing Society Chemotherapy Provider preferred; OCN preferred.
• Minimum two years hematology/oncology nursing experience; strong communication skills.

Responsibilities

• Act as liaison for patients, families and multidisciplinary team.
• Assess clinical, emotional, psychosocial, financial, and other patient needs.
• Coordinate discharge planning with family, social workers, home nursing, physicians.
• Educate patients and families on disease, treatments, side effects and signs.
• Develop patient education materials, clinical pathways and quality studies.
• Document recommendations from multidisciplinary conferences and tumor boards.
• Promote community education and cancer prevention awareness.
