NuWest is seeking an experienced Registered Nurse as a Clinical Care Coordinator to support gynecology and oncology patients in Newark, New Jersey. You will guide patients through their care journey, coordinating treatment, diagnostics, and follow-up while acting as a key link between patients, families, and the care team.

The ideal candidate is organized, compassionate, and confident handling complex cancer care plans, with strong communication and education skills for patients and families.

Qualifications

• Active RN license issued in New Jersey.
• Minimum 2+ years of Oncology experience.
• Must pass background checks and drug screening.

Responsabilités

• Coordinate patient care from diagnosis through treatment and follow-up.
• Serve as the primary clinical contact for patients and families, educating on diagnoses, plans, and side effects.
• Assess needs and symptoms, perform triage by phone and in person, escalate as needed to the provider.
• Collaborate with physicians and the care team to update individualized care plans.
• Review and manage referrals, test results, and authorizations to prevent delays.
• Educate patients on medications and self-care, reinforcing adherence.
• Connect patients with social work, financial counseling, nutrition, and palliative care resources.
• Document in the EMR and maintain HIPAA/compliance.
