Responsibilities

• Assesses and reassesses the patient and family for ongoing care needs and initiates referrals.
• Coordinates multidisciplinary care and schedules appointments for new patients.
• Collaborates with physicians, staff and caregivers to plan and implement a care plan.
• Educates patients and families about diagnoses, therapies and testing.
• Supports transitions of care and communication among care teams.
