Job summary

The APP will develop care plans and coordinate follow-up with a focus on quality and continuity of care.

The role requires a Master's degree (PA/NP) and PA/NP licensure in the applicable state, with preference for licensed providers and those with clinical experience.

Qualifications

• Master's degree in Physician Assistant or Nurse Practitioner required.
• Experience as licensed PA/NP preferred.
• State PA/NP license required.
• Skilled in taking medical histories and interpreting findings.
• Ability to communicate clearly with patients and staff.

Responsibilities

• Provides direct care, counseling, and teaching to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
• Performs complete histories and physical examinations.
• Orders, interprets, and evaluates laboratory and diagnostic tests.
• Develops appropriate plans of care and follow-up based on diagnostic results.
• Orders medications and writes prescriptions per policies.
• Maintains high-quality documentation including notes and discharge summaries.
• Performs bedside procedures as appropriate to patient population.
