Job summary

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens
• Provides patient and caregiver counseling on therapy, adverse effects, and adherence
• Reviews and interprets medication orders and participates in emergencies when applicable
• Supports medication safety and quality improvement activities
• Collaborates with care team to manage formulary adherence and medication-use processes
• Prepares, dispenses, and ensures proper storage and handling of medications
• Serves as preceptor and mentors students and residents
• Ensures compliance with policies, procedures, and regulations
