Qualifications

• Licensure in the state of practice or eligibility for reciprocity.
• Immunization and non-vaccine injectable certification as required.
• Credentialing and privileging based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs therapy regimens.
• Counsels patients and caregivers about medications and safety.
• Interprets medication orders accurately and reviews prescriptions.
• Supports medication safety, quality improvement, and formulary management.
• Prepares, dispenses, stores, and handles medications per regulations.
• Mentors students, residents, and pharmacy teammates as assigned.
• Ensures compliance with policies, procedures, and regulations.
