Job summary

Qualifications

• Bachelor’s or Doctorate degree in pharmacy from ACPE-accredited school
• Licensed in the state(s) of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification required
• Credentialing and privileging required based on area of practice
• Oncology/hospital/infusion experience preferred

Responsibilities

• Oversees pharmaceutical care and evaluates literature to design therapy regimens
• Provides counseling to patients and caregivers about therapy, adverse effects, and compliance
• Implements recommendations and documents in proper format
• Reviews and interprets medication orders and prescriptions
• Participates in management of medical emergencies when applicable
• Supports medication safety and quality improvement activities
• Performs quality assurance and improvement of medication-use system
• Collaborates with healthcare team for formulary adherence and medication-use process
