Advocate Health in Winston-Salem, NC seeks a Hematopoietic Transplant and Cellular Therapy Clinical Pharmacist to provide comprehensive pharmacotherapy and patient education in inpatient and clinic settings.

The role includes medication therapy design, order review, emergency participation, and precepting learners, with a weekend rotation and on-call duties. A sign-on bonus is available for eligible candidates.

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.
• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy or NABP FPGEC certificate.

Responsabilités

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Uses patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, compliance, and proper use.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens based on literature and policies.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in management of medical emergencies when applicable.
• Supports medication safety and quality improvement activities and reports events appropriately.
• Collaborates with the healthcare team to manage formulary adherence and medication-use process; assists in acquisition of non-formulary/compassionate/investigational meds.
• Ensures compliance with organizational policies and state/federal regulations.
• Prepares, dispenses, and oversees storage/handling of medications following best practices and regulations.
• Serves as preceptor for pharmacy students and residents; mentors and assists with performance evaluations.
• Directs and checks the work of pharmacy support staff to ensure safety and efficiency.
