Qualifications

Bachelors Degree in Pharmacy or Doctor of Pharmacy Degree.
Active Clinical Pharmacist license in the state of Illinois (IDFPR).
Preferred: completion of a pharmacy residency program or equivalent hospital pharmacy experience.

Responsibilities

Interprets medication orders to ensure safety and proper use of medications and coordinates solving of medication-related problems between pharmacy, nursing, and prescribers.
Monitors medication therapy for significant interactions and intervenes as needed.
Processes all medication order information into the EHR to ensure accuracy of records.
