Job summary

UW Health is seeking a Clinical Pharmacist in Oncology in Madison, WI to provide comprehensive medication management for cancer therapy, collaborating with the multidisciplinary oncology care team to optimize treatment outcomes.

You will deliver individualized patient and caregiver education on anticancer therapies, including treatment expectations, administration, adherence, toxicity monitoring, and self-management strategies.

Qualifications

• Bachelor's Degree in Pharmacy or PharmD required.
• ASHP Accredited Residency preferred.
• 1 year of clinical patient care and orders management experience required.
• 3 years institutional experience or residency preferred.

Responsibilities

• Provide comprehensive medication management for patients receiving cancer therapy in collaboration with the oncology care team.
• Deliver individualized education to patients and caregivers on anticancer therapies, including administration, adherence, toxicity monitoring, and self-management.
• Provide supportive care management for symptom prevention and treatment, including nausea, pain, infection risk, and adverse effects.
• Utilize delegation protocols to manage chemotherapy-related issues and monitor therapeutic drug levels.
• Serve as a preceptor and educator for pharmacy students and residents to develop clinical knowledge and critical thinking in oncology.
• Ensure appropriate utilization of medications to optimize patient outcomes.
