Job summary

UW Health is seeking a Clinical Pharmacist in Oncology to provide comprehensive medication management for cancer therapy, collaborating with the oncology care team to optimize outcomes. The role includes patient and caregiver education on anticancer therapies, including expectations, administration, adherence and toxicity monitoring.

You will also provide supportive care management for symptoms such as nausea, pain, and infection risk, and act as a preceptor to students and residents, fostering

Qualifications

PharmD or Bachelor’s in Pharmacy required.
Completion of ASHP accredited residency preferred.
1 year of clinical patient care and orders management required.
3 years hospital/institutional experience or ASHP residency preferred.

Responsibilities

Provide comprehensive medication management for patients receiving cancer therapy, collaborating with the oncology team to optimize treatment outcomes.
Deliver individualized patient and caregiver education on anticancer therapies, including expectations, administration, adherence, and toxicity monitoring.
Provide evidence-based supportive care management and education for symptom prevention and treatment.
