Job summary

Highmark Health is seeking a skilled Case Manager to deliver comprehensive case management services for members with complex health needs. You will assess, plan, coordinate, and monitor care while containing costs and ensuring quality outcomes across benefit contracts.

The role requires RN licensure and CCM certification within 36 months of hire; experience with diverse populations and CBT/DBT training are a plus. Office-based or remote position with travel <25%.

Qualifications

• 5 years in clinical, case/utilization management, and/or health insurance experience.
• Current State RN licensure or multi-state licensure via eNLC.
• CCM certification must be obtained within 36 months of hire; incumbents as of Aug 2026 are exempt.
• Preferred: specialty clinical experience (e.g., Oncology, Transplant, Maternity, NICU, Pediatric).
• Experience with diverse populations and cultural competency.

Responsibilities

• Manage a caseload of members with complex health needs.
• Conduct comprehensive assessments and develop complex care plans.
• Assess members’ health status, needs and resources.
• Coordinate care plans and ensure access to services.
• Monitor progress and adjust plans as needed.
• Document activities in compliance with policies and regulations.
• Provide guidance and mentorship to lower-level Case Managers.
