Job summary

Griffin Health is seeking a Cancer Services Patient Navigator to guide patients, families, and caregivers through the health care system from diagnosis to survivorship. The Navigator coordinates care with the oncology team within Griffin Health’s cancer program and accountable care framework to ensure timely, high-quality services.

Qualifications

• Bachelor's Degree in Nursing or graduate of an accredited nursing program with a Bachelor's Degree in a health related field.
• CT RN License, BLS, CPR, OCN, ACS LION or other Oncology Navigation certifications are required for this position.
• At least three or more years of experience working in an Oncology setting, knowledge of the Cancer Care Continuum and interacting collaboratively with the Oncology teams.

Responsibilities

• Provides supportive guidance and education for cancer patients, family members, and caregivers
• Facilitates communication, cooperation, and collaboration among health care providers
• Coordinates and supports delivery of Planetree Amenity Services including ensuring that Planetree amenity services staffed by volunteers are operating at established target levels
• Responsible for accurate record-keeping
• Maintains responsibility for attendance/reliability to ensure that the Hospital is operated in an efficient and cost effective manner
• Contributes to the mission of the organization by supporting the Employee Philosophy, Planetree Model of Care, and Organizational Goals
