Qualifications

• Bachelor degree from an accredited university and two years of healthcare experience, or high school diploma with 5 years in referral management, oncology navigation, or patient access.
• Strong knowledge of medical terminology and ability to work autonomously and in teams.
• Excellent interpersonal, organizational and communication skills.

Responsibilities

• Assist patients with screening and reinforce treatment plans, linking to health services and community resources.
• Serve as a patient liaison to help patients and caregivers schedule within the complex healthcare system.
• Advocate for care interventions identified by RN Navigator or Coordinator and address gaps in care.
