Job summary

Appalachian Regional Healthcare (ARH) seeks an oncology navigator to act as the main contact for patients and families across the cancer care continuum. You will collaborate with the care team to remove barriers, connect patients with needed services, and promote adherence to the care plan.

Responsibilities include building patient relationships, facilitating care coordination, and maintaining thorough documentation, with emphasis on rural Appalachian community needs and ongoing oncology

Qualifications

• Strong understanding of rural Appalachian culture.
• Proficient in computer use and medical record systems.
• Familiar with basic medical terminology and care processes.
• Excellent interpersonal communication with patients, families, and care teams.
• Flexible and able to adapt to changing care plans and workloads.
• Commitment to ongoing oncology navigation education and best practices.
• Encouraged to pursue recognized oncology navigation certifications.

Responsibilities

• Establishes close relationships with customers and serves as primary contact.
• Communicate with care team members to facilitate client care.
• Identify resources to overcome barriers to care (transportation, appointments, finances, psychosocial needs).
• Stay informed on services offered by providers, including mental health and housing resources.
• Participate in care planning meetings and tumor boards as needed.
• Document all client encounters per organizational standards and maintain confidentiality.
• Track attendance and outreach activities, initiating follow-up as necessary.
• Engage with outreach for prevention and screening opportunities and transitions of care.
