Job summary

Frederick Health is seeking a full-time Care Manager Social Worker for our Oncology Services. This role is scheduled Monday to Friday, 8:00 am to 4:40 pm or 8:30 am to 5:00 pm, and reports to the Department Manager.

The Care Manager Social Worker collaborates with physicians and healthcare professionals, assesses unmet social needs, provides information and referrals, conducts psychosocial assessments and counseling, and develops plans for managing identified needs across acute and ambulatory

Qualifications

• Graduation from an accredited school of social work with a Bachelors or Master’s degree in social work.
• Current Maryland social work license.
• Three to five years of care management and/or healthcare experience.
• Care Management certification preferred (ACM).
• CPR certification - not applicable for employees working in an approved, full time remote position.

Responsibilities

• Delivery of care coordination within Frederick Health which may include hospital or ambulatory site assignment.
• Performs psychosocial assessment and planning to develop the plan of care to address identified needs.
• Identification of barriers to care including socioeconomic status and unmet social needs.
• Promotes and values diversity to promote inclusive working relationships.
• Communication with interdisciplinary care team, patients, and families.
