AdventHealth is hiring a Care Manager to advocate for patients and families, coordinating discharge planning and post-acute care to ensure timely progression of care.

The role requires strong communication, critical thinking, and knowledge of social work and care management. You will collaborate with multidisciplinary teams, review medical records, arrange post-acute services, and educate families on resources and options.

Qualifications

• Bachelor's degree required
• Master's preferred
• Experience in social work or care management preferred
• Strong communication and negotiation skills required
• Ability to work with diverse populations and complex care needs
• Familiarity with discharge planning and post-acute services preferred
• Knowledge of community resources and utilization management preferred
• Ability to educate patients and families about care options and resources

Responsibilities

• Advocate for patients and families during hospitalization
• Coordinate discharge planning and post-acute care arrangements
• Communicate with payors for authorization needs
• Document discharge planning evaluations and barriers to progression
• Review medical records and integrate clinical, social, and financial factors into care plans
• Develop contingency discharge plans to ensure timely transitions
• Participate in multidisciplinary rounds and coordinate community resources
• Educate patients and families about resources, options, and rights
