Job summary

Frederick Health is seeking a Care Manager Social Worker to integrate into interdisciplinary teams across ambulatory and hospital settings. The role focuses on psychosocial assessments, resource referrals, and care planning to improve outcomes and resource utilization.

Responsibilities include coordinating care, addressing barriers, and educating patients and families while supporting navigation through the healthcare continuum.

Qualifications

• Graduation from an accredited school of social work with a Bachelors or Master’s degree in social work and a current Maryland license.
• Master’s Degree preferred.
• Three to five years of care management and/or healthcare experience.
• Care Management certification preferred (ACM).
• CPR certification required only outside remote positions and CEU requirements must be kept up to date.

Responsibilities

• Deliver care coordination across hospital and ambulatory sites.
• Conduct psychosocial assessments and develop plans to address identified needs.
• Identify barriers to care including socioeconomic factors and support networks.
• Coordinate with interdisciplinary care team, patients, and families.
• Monitors and evaluates care plans and provides education.
