Job summary

Connecticut Oncology Group is seeking an Advanced Practice Provider to join its hematology and oncology team in a collaborative outpatient setting. The APP will work closely with physicians to manage patients across the cancer care continuum, including follow-up visits, treatment monitoring, and symptom management.

The position involves practicing three days per week at our Middletown clinic and one day per week at our Westbrook clinic, fostering teamwork and delivering high-quality,

Qualifications

• Minimum of 1-2 years of AP experience required.
• Hematology/Oncology experience as RN or AP preferred.

Responsibilities

• Manage patients across the cancer continuum, including follow-up visits and treatment monitoring.
• Provide symptom management and care coordination in a collaborative outpatient setting.
• Work three days per week in Middletown and one day per week in Westbrook to support both locations.
