Job summary

Qualified candidates hold a DE CNA certification, have reliable transportation, and can work independently as well as within a multidisciplinary team in a home or community setting.

Qualifications

• Certified Nursing Assistant in DE (state certification) is required.
• Driver's License and access to reliable transportation.
• 1+ years of hospice care, home health, long-term care, nursing homes, or assisted living experience.
• Ability to work independently and as part of a multidisciplinary team in a home or community setting.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs): bathing, dressing, grooming, and mobility.
• Monitor and document vital signs and changes in condition.
• Communicate with the healthcare team regarding patient status and needs.
• Provide emotional support to patients and families, respecting cultural preferences and dignity.
