Job summary

Catholic Charities Eastern Washington is seeking a Care Coordinator to provide supportive housing services for households re-entering the community. You will collaborate with hospital teams, housing partners, and service providers to develop individualized plans and facilitate smooth transitions.

The role requires engagement with clients, landlords, and community resources to promote independence and stability, while adhering to program requirements and ethical guidelines.

Qualifications

• BA in Psychology, Social Work, or equivalent from an accredited institution.
• Lived experience with chemical dependency or mental health disorders preferred.
• Minimum of two years’ direct treatment experience with mentally ill persons.

Responsibilities

• Provide supportive housing care coordination for households discharging from Eastern State Hospital or qualifying under MTD.
• Coordinate with landlords and treatment teams during tenancy to address issues proactively.
• Develop client-centered service plans and assist with housing searches and related documentation.
• Maintain HIPAA-compliant records and timely documentation to support care outcomes.
