Job summary

Catholic Charities of Santa Clara County in San Jose is seeking a Supportive Housing Services Coordinator to broker access to housing, food, medical, educational, and other critical services for CHDC residents. The role collaborates with CHDC property management to support safe, stable housing and to document services in the electronic health record.

The coordinator conducts intake interviews, coordinates workshops, assists with referrals, and tracks HMIS data.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree in social work, human services, psychology, or related field preferred or equivalent experience.
• Minimum of two years’ experience in social services (housing services or case management preferred).

Responsibilities

• Provides site with monthly events calendar, newsletter, and workshops per contract requirements.
• Conducts intake interviews and introduces services to new tenants.
• Communicates with property manager to assess needs and promote safety; arranges disaster or emergency exercises.
• Assists with referrals to community resources and client consultations.
• Aids clients with applications for financial assistance and housing stability.
• Advocates for tenants with property managers and service providers.
• Promotes daily life skills and social activities.
• Maintains client files and documents in EHR; records HMIS data as required.
