EmpowerMe Wellness is seeking a Care Coordinator to support on-site teams in senior living communities. The role focuses on scheduling, intake, and care coordination touchpoints with residents, families, and staff to ensure high-quality care.

The ideal candidate will be highly organized, detail-oriented, and capable of multitasking within a fast-paced healthcare environment. Strong communication and collaboration with therapists, directors, and caregivers are essential.

Qualifications

• Certification in healthcare, business administration, or related field.
• Two years of relevant experience in a senior living setting preferred.
• Experience in administrative and operational roles managing schedules and records.
• Excellent communication and interpersonal skills with residents and families.

Responsibilities

• Identify seniors’ health needs by building caseload and coordinating screenings.
• Support intake processes including gathering documents and consents.
• Provide care coordination services through regular touchpoint visits and wellness activities.
• Assist in scheduling evaluations and coordinating with clinical staff.
• Coordinate therapists’ weekly schedules and onboard new team members.
• Review operational reports and support billing closeouts.
• Maintain HIPAA/PHI compliance and professional relationships with care teams.
