Job summary

Infinite Medical P.C. is seeking a licensed nurse (RN or LPN) for an on-site Nursing Care Coordinator role to ensure chart documentation compliance and continuity of care for residents in skilled nursing and long-term care facilities.

You will review records, coordinate with HR and clinical teams, and maintain proficiency across EMR platforms to support high-quality care and regulatory adherence. This is a full-time daytime position with benefits and a $1,000 sign-on bonus.

Qualifications

• Licensure: RN or LPN with valid credentials.
• Experience with geriatric care and chronic conditions.
• Full-time, on-site schedule: Mon–Fri, 9:00 AM–5:30 PM.

Responsibilities

• Review patient records and RVLs to ensure CMS compliance.
• Identify documentation discrepancies and propose corrections.
• Coordinate with HR, Operations and Clinical teams to resolve inquiries.
• Participate in mandatory operational/educational meetings and cross-training.
• Maintain proficiency across primary EMR platforms and digital tools.
