Job summary

Infinite Medical P.C. is seeking a detail-oriented Licensed Nurse (RN or LPN) to serve as a Nursing Care Coordinator on-site. The role focuses on chart review, compliance, and care management to ensure seamless continuity for residents in skilled nursing facilities.

The position offers a full-time, weekday schedule (Mon–Fri, 9:00 AM – 5:30 PM) with a $30.00–$34.00 per hour pay range, a $1,000 sign-on bonus, and benefits including a 401(k), medical/dental/vision, and life insurance.

Qualifications

• RN or LPN licensure required and active.
• Experience with geriatric care and chronic conditions preferred.
• On-site, full-time schedule with daytime hours (Mon–Fri 9:00–5:30).

Responsibilities

• Review patient charts for completeness and regulatory compliance.
• Identify documentation discrepancies and propose corrections.
• Coordinate with HR, Operations, and Clinical teams to resolve inquiries.
• Attend mandatory meetings and cross-train in care management.
• Maintain proficiency with EMR platforms like PCC, MatrixCare and SigmaCare.
