Job summary

Infinite Medical P.C. is seeking a detail-oriented on-site Licensed Nurse (RN or LPN) to serve as a Nursing Care Coordinator. You will review and verify chart documentation to ensure CMS compliance and seamless continuity of care for our patient population.

The role requires meeting daily productivity goals, strong documentation skills, and coordination with HR, Operations, and Clinical teams. On-site schedule is Monday–Friday, 9:00 AM–5:30 PM, with a comprehensive benefits package.

Qualifications

• Licensed RN or LPN with current licensure.
• Experience with geriatric care and chronic disease management.
• On-site full-time schedule Monday–Friday, 9:00 AM–5:30 PM.
• Strong documentation and CMS guideline knowledge.

Responsibilities

• Compliance & Records Review: evaluate patient records and RVLs for accuracy and CMS compliance.
• Quality Assurance: identify discrepancies, generate corrections, and suggest resolutions.
• Cross-Functional Communication: coordinate with HR, Operations, and Clinical teams to resolve inquiries.
• Professional Development: participate in mandatory meetings and cross-train in care management.
• Systems Utilization: maintain proficiency across multiple EMR/digital platforms.
