Infinite Medical P.C. is seeking a on-site Nursing Care Coordinator (RN or LPN) to review and verify chart documentation, ensuring CMS compliance and continuity of care in a skilled nursing setting. The role emphasizes administrative accuracy and collaboration with care teams.

Commitment to a full-time daytime schedule and mastery of EMR platforms is essential, with a focus on quality and efficient documentation. Competitive hourly compensation and benefits are provided.

Qualifications

• Valid RN or LPN license in NY.
• Full-time on-site schedule: Mon-Fri, 9:00 AM–5:30 PM.
• Experience with LTC/SNF care coordination and CMS guidelines.

Responsibilities

• Compliance & Records Review: verify charts and RVLs for accuracy and regulatory adherence.
• Quality Assurance: identify discrepancies and suggest corrections.
• Cross-Functional Communication: coordinate with HR, Operations, and Clinical teams.
• Professional Development: attend mandatory meetings and cross-train in care management areas.
• Systems Utilization: use PCC, MatrixCare, SigmaCare, Reliable/Visual, Yardi-One and Google Workspace.
