Job summary

Holton Manor is seeking a compassionate Physical Therapist Assistant (PTA) to join our skilled nursing and long-term care team in Elkhorn, Wisconsin. This on-site position supports residents in improving mobility and independence under supervision of a licensed Physical Therapist.

The PTA will implement treatment plans, maintain documentation, and collaborate with the interdisciplinary team to enhance quality of life for residents across care settings.

Qualifications

• Graduate of an approved PTA education program.
• Licensed, if applicable, in the state of practice.
• Uphold professional, legal and ethical standards of physical therapy.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Supervise and mentor LPNs and CNAs.
• Ensure infection control and safety compliance.
• Collaborate with the interdisciplinary care team.
