Holton Manor in Elkhorn, WI is seeking a licensed physical therapist to provide skilled therapy services across inpatient, outpatient and home health programs. The role supports an in-house therapy team of PT, OT, and SLP, with seven-day coverage and a focus on consistent patient care in a community-based setting.

The position offers flexible scheduling (days and PMs, with PRN options), on-site work, comprehensive benefits, ongoing education, and opportunities to grow within a 60-bed facility

Qualifications

• Graduate of an accredited physical therapy program.
• Wisconsin licensure required.
• Ability to meet health, background check, and training requirements.
• Strong communication and multidisciplinary collaboration.

Responsibilities

• Perform initial patient examination and develop plan of care.
• Maintain patient records and documentation.
• Provide on-site assessment and re-evaluation per policy.
• Coordinate discharge plans with the PTA.
• Ensure compliance with state practice act and ethical standards.
