Job summary

Holton Manor in Elkhorn, WI seeks a Physical Therapist to support inpatient, outpatient and home health programs. The role is on-site with flexible scheduling across days, PMs and Saturdays, and PRN options available to fit your availability.

You will work with a dedicated therapy team in an in-house program, providing skilled therapy, documentation, and collaboration with occupational and speech colleagues. Wisconsin license and graduation from an accredited PT program are required.

Qualifications

• Graduate of an accredited education program in physical therapy.
• Wisconsin license in good standing.
• Ability to work collaboratively within a hospital/therapy department and adhere to policies.

Responsibilities

• Uphold professional, legal, and ethical standards for physical therapy per APTA and state act.
• Ensure delivery of quality services and outcomes.
• Perform initial patient examinations and develop plans of care.
• Maintain patient records for treated patients.
• Re-evaluate and adjust treatment plans as needed.
• Coordinate discharge planning with PTA and care team.
• Document according to facility policies and ensure medical necessity.
