Holton Manor is seeking a compassionate Physical Therapist Assistant (PTA) to join our skilled nursing and long-term care team on-site in Elkhorn, WI. The PTA will implement treatment plans under an licensed Physical Therapist and help residents improve mobility and independence.

The role supports documentation, collaboration with the interdisciplinary team, and ongoing professional development in a supportive community focused on quality of life.

Qualifications

• Graduate of an approved education course for physical therapy assistants.
• Licensed, if applicable, in the state in which they will be practicing.
• Uphold professional standards of practice for physical therapy.
• Maintain patient records and care documentation.
• Perform selected PT interventions under supervision.

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.

Job Summary:

The Physical Therapist Assistant (PTA) works under the direction and supervision of a licensed Physical Therapist to implement individualized treatment plans for residents in a skilled nursing and long‑term care setting. The PTA assists residents in achieving mobility and rehabilitation goals while maintaining accurate documentation and collaborating closely with the interdisciplinary healthcare team.

Qualifications:

• Must uphold the professional, legal and ethical standards of practice for physical therapy as defined by the APTA and State Practice Act
• Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department.
• Creates and maintains a patient record for each patient treated
• Performs selected physical therapy interventions under the direction and supervision of the physical therapist.
• Obtains data related to the interventions provided
• Makes modifications in selected interventions either to progress the patient/client as directed by the physical therapist or to ensure patient/client safety and comfort.
• Carries out the d/c plan as determined by the PT
• Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalitiesCompletes documentation according to facility policies including timeliness and medical necessity support
• Maintains communication with the supervising PT
• Participates in supervisory visits with the supervising PT as required by the State Practice Act
• Requests re-examinations from the supervising PT when a change in patient status is identified
• Promote resident dignity, independence, and quality of life in all interactions
