Qualifications

Responsibilities

• Maintain and update the patient pool for the Clinical Pharmacist Program.
• Call patients for appointment reminders and reschedule.
• Enrol patients referred by providers into pharmacist-led protocols.
• Schedule patients per clinic workflows and EMR protocols.
• Document outreach attempts and scheduling outcomes.
• Provide weekly updates on outreach and enrollment.
