Job summary
Circadia Health in Mt. Juliet, TN is seeking an on-site Nurse Practitioner to transform bedside care with AI, seeing patients first, acting on real-time signals, and coaching facility teams.
This full-time role blends direct clinical work with tech-to-bedside integration through hands-on training, live demos, and joint assessments, with a focus on reducing readmissions and improving patient outcomes. You will work Monday–Friday at Cedar Creek Post Acute facility, collaborating with clinical ops
Qualifications
- Licensed NP in MI with ANCC in relevant specialty.
- 2+ years NP experience with complex adult populations.
- Eligible for Medicare participation; company bills on your behalf.
- Valid MI driver's license; able to travel locally.
Responsibilities
- Direct patient care: initial and follow-up assessments; plan of care adjustments.
- Tech → bedside integration: coach staff and standardize Circadia use.
- Data into action: interpret trends, prioritize high-risk patients, intervene.
- Close the loop: complete notes within the same day.
- High-volume rounding: 25+ visits/day when appropriate.
- Quality & product feedback: surface insights to improve workflows.
- Mentor & collaborate: support NP onboarding with clinical ops/product teams.
