You will perform initial and follow-up assessments, train staff on Circadia tools, and translate trend data into timely interventions to reduce readmissions.

This full-time role offers a salary range of $130,000–$160,000 with comprehensive benefits and opportunities to shape care decisions for seniors.

Qualifications

• Licensed Nurse Practitioner in Louisiana (ANCC or equivalent).
• Eligibility for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• English language proficiency required.

Responsibilities

• Direct patient care: assess residents and adjust plans of care.
• Tech to bedside integration: train staff and integrate Circadia into workflows.
• Interpret trend-based insights and prioritize high‑risk patients.
• Complete charts/notes on the same day.
• Conduct high-volume rounds across SNFs (≈25+ per day).
• Provide feedback to improve workflows and product integration.
• Mentor NP onboarding and collaborate with clinical ops/product teams.
