Job summary

CareCore at Lakeridge is seeking a dedicated full-time MDS Coordinator to join our team on-site in Ohio. The role focuses on ensuring MDS compliance and managing RUGs/PDPM, with emphasis on accuracy, quality measures, and timely documentation.

The ideal candidate is a licensed nurse in Ohio (RN preferred) with at least 1 year of MDS experience, strong knowledge of MDS 3.0 guidelines, and proficiency in EHR/MDS software. Collaboration and excellent organizational skills are essential.

Qualifications

• Licensed Nurse in the State of Ohio (RN Preferred).
• RAC-CT Certification Preferred.
• Minimum of 1 year of experience as an MDS Coordinator or equivalent role.
• Strong understanding of MDS 3.0 guidelines and skilled nursing facility regulations.
• Knowledge of the Medicare and Medicaid reimbursement process.
• Excellent communication, organizational, and time management skills.
• Proficiency in electronic health records (EHR) and MDS software systems.
• Ability to work collaboratively in a team-oriented environment.

Responsibilities

• Ensure compliance with federal and state guidelines related to the MDS.
• Manage Medicaid and Medicare RUGs/PDPM, Medicaid Case Mix, and Medicare criteria.
• Prepare and maintain required supportive documentation.
• Collaborate with interdisciplinary teams to optimize resident care.
• Utilize EHR and MDS software to document and track data.
