Job summary

Ruby's Place dba Restorative Pathways (RP) in Alameda, CA is seeking a Case Manager for the ESH Program. This full-time, on-site role offers 40 hours per week of non-exempt work in a community-focused nonprofit setting.

The Case Manager will provide comprehensive on-site case management, housing stabilization, and resource connection for families and individuals experiencing housing instability, following Housing First principles and survivor-centered practices.

Qualifications

• Minimum 2 years of experience in case management, housing navigation, or homeless services.
• Experience working with families and individuals experiencing housing instability.
• Knowledge of Housing First principles and local housing resources.
• Strong written and verbal communication skills.
• A valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver to drive agency vehicles.
• Ability to complete all required background checks including fingerprinting and licensing requirements.

Responsibilities

• Conduct intake assessments and develop individualized, goal-oriented case plans.
• Collaborate with participants to identify housing barriers and stability needs.
• Facilitate housing navigation, including rental applications, landlord engagement, and housing search support.
• Support participants in obtaining necessary documentation (ID, income verification, benefits).
• Provide referrals and warm handoffs to community-based services (employment, healthcare, behavioral health, childcare).
• Monitor participant progress toward housing and stability goals.
