In this field-based position in Tacoma, you will support patients transitioning from hospital or skilled nursing facility discharge. Responsibilities include post-discharge follow-up, medication reconciliation, care coordination, and patient education. The ideal candidate must have completed an accredited MA program and hold current certification. Strong communication skills and the ability to work independently are essential for success.

Responsabilidades

• Post-discharge patient outreach and follow-up
• Medication reconciliation and care coordination
• Obtain vitals and clinical updates (within MA scope)
• Educate patients and caregivers on care plans
• Collaborate with providers, facilities, and home health agencies
• Timely EMR documentation
• Travel to facilities and patient homes across Washington State

Conocimientos

Comfortable working independently in the field

Educación

Completion of accredited MA program

Current certification required
