Job summary

Adaptive Home Health, LLC is seeking a Clinical Liaison to enhance patient transitions from facilities to home health services. This full-time role will involve building strong relationships with case managers and conducting bedside visits to ensure a smooth referral process.

Ideal candidates will possess an active Texas license, strong clinical knowledge, and the ability to engage effectively with patients and facility staff.

Qualifications

• Must have an active, unrestricted Texas license as an LVN/LPN, PTA, or COTA.
• Minimum of 1 year clinical experience in home health or rehabilitation setting.
• Strong interpersonal and communication skills are essential.

Responsibilities

• Serve as the daily on-site presence at the assigned facility.
• Identify patients appropriate for home health services.
• Coordinate with the Account Executive on facility-specific strategy.
