Adaptive Home Health is looking for a Clinical Liaison to connect facilities with our home health services. This full-time role will require you to be present at the assigned facility, building trust and relationships with discharge planners and case managers.

You will educate patients on home health services and manage referrals, ensuring a smooth transition from facility care. The position offers a competitive salary of $65,000 - $80,000 along with benefits and a supportive operational team.

Qualifications

• Must have active, unrestricted Texas license as LVN/LPN, PTA, or COTA.
• Minimum of 1 year of clinical experience in home health or similar setting.
• Strong interpersonal and communication skills required.

Responsibilities

• Serve as the daily on-site presence at assigned facility.
• Identify patients suitable for home health services through active engagement.
• Conduct bedside visits with patients pre-discharge.
