Job summary

The Pennant Group, Inc. is seeking a Clinical Liaison to manage the on-site Healthcare Clinic and collaborate with the team to improve patient care outcomes and the company profile. The role involves supervising aides and caregivers and making independent clinic decisions.

Responsibilities include resident assessments, coordination with physicians, end-of-day reporting, and participation in IDG planning. A nursing license and CPR certification are required, with at least three years of nursing

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; community/home health or medical/surgical experience preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.

Responsibilities

• Manages all aspects of the Healthcare Clinic including appointments and inventory.
• Performs assessments on residents in the facility.
• Evaluates residents for services such as hospice, home health and homecare.
• Interacts with residents, physicians and providers to ensure optimal patient outcomes.
• Prepare and submit end of day reports to Clinical Management and Business Development Manager.
• Manages and delegates to the aide/caregiver(s).
• Provides direct patient care as defined in Idaho Nurse Practice Act.
• Participates in IDG meetings to keep the plan of care current.
• Supports marketing and community outreach within plan and timetable.
• Assists in development and implementation of a marketing plan.
