Job summary

Hamilton Health Box is seeking an On-Site Clinicial Assistant to support daily clinical and administrative operations across Houston-area locations. You will interact with patients for scheduling, deliver protocol-driven care, and help coordinate care with pharmacy services and referral networks.

The role emphasizes patient communication, documentation in EMR, and participation in health education initiatives.

Responsibilities

• Interact with patients for scheduling and follow-up, delivering protocol-driven care.
• Coordinate care delivery with pharmacy services and referral networks.
• Document in patient medical records and manage data entry in EMR systems.
• Assist with preventative screenings and health education initiatives.
• Provide one-on-one counseling and health navigation for clients.
• Communicate barriers to care to the Pod Manager and support training.
